My great-grandfather (my father's mother's father).
We're currently unsure of the spelling of the last name Potiahaylo. It was changed arbitrarily to Pothalo, Potahailo or Puthalo (we're not sure) by the immigration authorities and that family name was used on the children's birth certificates.

Born February 1891 in the village of Sarafinchi (alternate spellings: Serafince, Serafin'tse, Serafińce, Serafintsy, Serafin), birth date unknown. This is a village near the city of Horodenka (also seen as Gorodenka, Ukrainian: Городенка) in what is now the Ivano-Frankivsk Oblast of Ukraine.
